Troponine I hyper sensible : marqueur spécifique d'une nécrose myocardique, mais non spécifique de l'IDM.
L'ESC recommande un intervalle de 3h entre 2 dosages afin de distinguer un IDM d'une atteinte cardiaque chronique (Hamm CW et al., Eur heart J 2011).
Pour un début des symptômes < 8H : Tropo < 0.02 µg/l : IDM exclu à 99%
Tropo 0,02 - 0,04 µg/l : VPP pour l'IDM à 62%
Tropo > 0,04 µg/l : VPP pour l'IDM à 71%. Prise en charge cardiologique nécessaire.
Dans tous les cas, un nouveau dosage à 3 h d'intervalle permet d'affirmer ou infirmer un nécrose myocardique.
