La diminution est observée dans l'insuffisance hépato-cellulaire mais aussi dans les fuites protéiques et dans l'avitaminase A. Son augmentation, corrélée avec celle de l'haptoglobine, évoque une inflammation. A remarquer que l'augmentation de l'orosomucoïde précède celle de l'haptoglobine et est donc utile pour dater l'inflammation.
